SeaSwift Passenger Medical Information Form
MV “Trinity Bay” requires passengers to complete a Passenger Medical Form to advise the crew of any medical issues they may need to be aware of. This information will also be used by medical attendants in the event of a medical emergency or accident.
The vessels is a working industrial ship and travels to remote locations. There is no doctor on board and it can be up to a day from professional medical assistance so all passengers are required to truthfully and accurately complete and return a medical questionnaire prior to the confirmation of a booking. Certain conditions will necessitate a Doctor’s medical clearance.
There is no doctor on the vessel, only trained first-aid personnel.
Passengers should consider their suitability for travel to a remote area and for managing cargo-ship conditions such as: engine noise, steep staircases, high door step-overs, awkward gangway access, the possibility of a cabin without a porthole and the possibility of rough weather or a potential emergency situation. An appropriate level of health and confident, independent mobility is essential.
Most cabins have bedding for 3 or 4 passengers and contain at least one 2-level bunk-bed. Where more than 2 passengers are sharing a cabin, the 3rd and 4th passenger will need to climb onto an upper bunk.
