Seaswift Medical Form - Sea Swift Pty Ltd ("Sea Swift")
Sea Swift vessels are working industrial ships and travel to remote locations. There is no doctor on board and we can be up to a day from professional medical assistance so we require all passengers to truthfully and accurately complete and return this medical questionnaire prior to the confirmation of a booking. Certain conditions will necessitate a Doctor’s medical clearance.
IMPORTANT: Please note that MV “Trinity Bay” requires passengers to complete a Passenger Medical Information Form to advise the crew of any medical issues they may need to be aware of. This information will also be used by medical attendants in the event of a medical emergency or accident.
